Trailhead Treatment Center is required by federal law to maintain the privacy of your protected health information (PHI), provide you with this notice of our legal duties and privacy practices, and follow the terms of this notice currently in effect.
As a substance use disorder and mental health treatment provider, your records are also protected under federal confidentiality regulations (42 CFR Part 2), which provide additional protections beyond HIPAA for substance use disorder treatment records.
We may use your health information to provide, coordinate, or manage your treatment and related services. This includes sharing information among your treatment team members (therapist, case manager, clinical director, prescribing providers) to ensure coordinated care.
We may use and disclose your health information to obtain payment for treatment services, including submitting claims to your insurance company, verifying coverage, and obtaining prior authorization.
We may use your health information for activities necessary to operate our facility, including quality assessment, staff training, compliance auditing, and business management.
Federal law (42 CFR Part 2) provides additional protections for information about substance use disorder treatment. Under these regulations:

You have the right to inspect and obtain a copy of your health information maintained by Trailhead. Requests must be submitted in writing.
You have the right to request that we amend your health information if you believe it is incorrect or incomplete.
You have the right to request a list of disclosures of your health information that we have made, other than for treatment, payment, or healthcare operations.
You have the right to request that we restrict how we use or disclose your health information for treatment, payment, or healthcare operations.
You have the right to request that we communicate with you about your health information in a specific way or at a specific location.
You have the right to obtain a paper copy of this notice upon request.
