Trailhead Treatment Center in Salem, NH provides outpatient treatment for substance use disorders and mental health conditions including alcohol addiction, opioid addiction, drug addiction, trauma, anxiety, and depression. Our clinical team holds dual licensure from the State of New Hampshire for both SUD and mental health treatment, which means one team addresses both conditions in the same program, at the same time.
Trailhead treats all levels of substance use through evidence-based clinical care combined with holistic therapies. Your treatment plan starts with a clinical assessment using ASAM, LOCUS, and K10 tools to determine the right level of care.
Alcohol use disorder is one of the most common reasons people contact Trailhead. Our team offers individual therapy, group therapy, and medication-assisted treatment with Vivitrol (a monthly injection that reduces alcohol cravings). Treatment is available at all program levels: PHP, IOP, and outpatient. Co-occurring mental health conditions like anxiety and depression are treated simultaneously under the same clinical team.
Trailhead offers three injectable MAT medications for opioid use disorder: Vivitrol, Sublocade, and Brixadi. All three are monthly injections, eliminating the daily compliance burden of oral medications. MAT can be combined with PHP, IOP, or individual therapy, or received as a standalone service. Our team also collaborates with external methadone clinics for clients who need methadone.
Addiction does not follow one pattern. Trailhead treats substance use disorders involving stimulants (cocaine, methamphetamine, prescription stimulants), benzodiazepines (Xanax, Klonopin), prescription medications, and polysubstance use. Each client receives a treatment plan built around their specific substances, history, and co-occurring conditions. Medication-assisted treatment is available for opioid and alcohol components.

Co-occurring disorders (also called dual diagnosis) means a substance use disorder and a mental health condition are present at the same time. Trailhead’s integrated model treats both simultaneously under one clinical team. Most outpatient facilities in New Hampshire are licensed for SUD or mental health. Trailhead holds both licenses, so there is no gap in care, no referral to a second provider, and no “get sober first” approach.
Unresolved trauma drives substance use for many of the people who come to Trailhead. Our clinical team uses trauma-focused therapy as a primary modality for trauma and PTSD, along with movement-based therapies, yoga, breathwork, and grounding techniques. Trauma-informed care runs through all programming at every level, not as a separate track.
Anxiety disorders frequently co-occur with substance use. Trailhead’s treatment approach focuses on self-regulation: learning to manage anxiety, stress, and emotional dysregulation through evidence-based therapy and holistic practices like breathwork, meditation, and yoga. Individual therapy, group therapy, and medication management are all available.
Depression and substance use often reinforce each other. Trailhead addresses both at the same time through individual therapy, group therapy, and medication management. Holistic therapies including yoga, meditation, nature-based activities, and sound therapy support recovery from depression alongside clinical treatment.

Trailhead accommodates individuals with high-functioning autism spectrum conditions and those with a history of eating disorders. These are not specialty programs; our clinical team integrates appropriate accommodations into each client’s treatment plan.
Trailhead is an outpatient facility and cannot serve individuals who require inpatient or residential level of care.
