Trailhead Treatment Center offers an intensive outpatient program (IOP) in Salem, NH for adults managing addiction, substance use disorders, and co-occurring mental health conditions. Our outpatient addiction treatment program runs five days per week with daytime sessions from 9:00 AM to 12:30 PM, and evening options are coming soon. Trailhead is dual licensed by New Hampshire for both substance use and mental health treatment.

IOP at Trailhead is built for working adults and students who need structured clinical support without stepping away from daily responsibilities. You keep your job and come home to your family every day.

What Is an Intensive Outpatient Program?

An intensive outpatient program (IOP) is an outpatient addiction and mental health treatment program where clients attend group therapy and clinical services five days per week for several hours per session, then go home the same day. IOP sits between a partial hospitalization program (PHP) and standard outpatient therapy in intensity.

At Trailhead, IOP is group therapy-focused with integrated case management, holistic programming, and access to medication-assisted treatment when appropriate.

IOP Schedule and Format

Daytime IOP

Days: 5 days per week

Hours: 9:00 AM to 12:30 PM

Format: Group therapy, case management, holistic programming

Sessions end before 1:00 PM, so you have afternoons free for work, childcare, or school. Your employer does not need to know.

Evening IOP

Evening scheduling is planned and coming soon. We will announce days, times, and start date once confirmed.

What Is Included in IOP at Trailhead

Each client is assigned an individual therapist and case manager. Your therapist stays with you throughout treatment, including if you step up to PHP or down to outpatient.

Women-Only Track

Women-Only Track Within IOP

Trailhead offers a women-only track within our intensive outpatient program. This is the first women-only outpatient treatment track in New Hampshire.

The women’s track follows the same clinical structure as standard IOP with group content focused on trauma, codependency, and empowerment. Participants have dedicated group sessions in a women-only setting while attending alongside the broader IOP schedule.

This track is available in both IOP and our Partial Hospitalization Program.

Flexible Pathways

IOP Within a Full Range of Programs

You do not have to switch providers or start over if your needs change. Trailhead offers multiple levels of outpatient care under one roof with the same treatment team.

Step down from PHP: Clients completing our PHP transition directly into IOP with the same therapist. No new intake. No repeating your story.

Direct admission: If your clinical assessment shows IOP as the right level of care, you start here.

Step up if needed: Your clinical team can move you into PHP without leaving Trailhead.

Step down to outpatient: After IOP, continued care through our Outpatient Program keeps the same treatment team in place.

Most people start with a phone call to our admissions team. From there, we figure out the right program and schedule together.

All IOP sessions are currently in person at our Salem, NH facility.

Trailhead was founded by Caitlin and Mike DiBlasi — people who know from firsthand experience what it feels like to sit in the chair you are sitting in. They also know what recovery asks of a family, because they have lived that too.

Our staff have recovery experience alongside clinical training. Clinical Director Alysia LaGambina, LICSW, has spent years in adult mental health and addiction treatment across outpatient and public health settings. Her approach is holistic, trauma-informed, and built on the connection between mental health, environment, and lived experience.

Trailhead is dual licensed by the State of New Hampshire for both substance use and mental health treatment. We treat both conditions with one integrated team.
