Treatment Programs at Trailhead

Trailhead Treatment Center in Salem, NH provides outpatient addiction and mental health treatment programs for adults at every stage of recovery. Our programs include a partial hospitalization program (PHP), intensive outpatient program (IOP), standard outpatient services, medication-assisted treatment (MAT), mental health treatment for co-occurring disorders, and family support services. Trailhead is dual licensed by New Hampshire for both substance use disorder and mental health treatment.

Every program at Trailhead is recovery-owned. Co-founders Caitlin and Mike DiBlasi built these programs around what they wished had existed when they needed help — shaped by firsthand experience, not theory. Clinical Director Alysia LaGambina, LICSW, leads a trauma-informed clinical team that treats addiction and mental health simultaneously with one integrated care plan.

Programs Overview

Our Treatment Programs

Trailhead offers a full continuum of outpatient care. Each program is designed for a different level of need, and your clinical assessment determines the right starting point. You can step up or step down between programs without switching providers or repeating your story.

Partial Hospitalization Program (PHP)

The most intensive outpatient program we offer. PHP meets five days per week, 9:00 AM to 3:30 PM, with a minimum of five clinical hours per day. Programming includes individual therapy, group therapy, holistic therapies (yoga, reiki, breathwork, meditation), case management, injectable MAT, and lunch provided daily.

A women-only track is available within PHP, with group content focused on trauma, codependency, and empowerment. This is the first women-only outpatient treatment track in New Hampshire.

PHP is designed for adults who need structured daily support but do not require residential or inpatient care.

Intensive Outpatient Program (IOP)

Structured treatment that fits around work, school, and family. Daytime IOP meets five days per week, 9:00 AM to 12:30 PM. An evening IOP schedule is coming soon. IOP is group therapy-focused with case management, holistic programming, and access to injectable MAT.

The women-only track is also available within IOP.

Clients can enter IOP directly or step down from PHP with the same therapist and treatment team.

Outpatient Program

Continued recovery support with flexible scheduling. Our outpatient program provides individual therapy sessions and medication management for adults stepping down from IOP or seeking ongoing structured support without group-intensive programming.

Your treatment team stays the same when you step down from a higher level of care. No new intake. No repeating your history.

Medication-Assisted Treatment (MAT/MOUD)

Injectable medications combined with therapy for opioid and alcohol use disorders. Trailhead offers Vivitrol, Sublocade, and Brixadi with three service options: injection paired with IOP, injection paired with individual therapy, or standalone injection for clients in stable recovery.

Vivitrol treats both opioid use disorder and alcohol use disorder. Trailhead also collaborates with external MAT providers, including methadone clinics, so clients can receive other medications while attending our programs.

Mental Health Treatment

Integrated treatment for co-occurring mental health and substance use disorders. Trailhead treats trauma and PTSD, anxiety, depression, emotional dysregulation, stress-related conditions, and co-occurring disorders. Treatment modalities include trauma-focused therapy, CBT, movement-based therapies, and trauma-informed care across all programming.

Because Trailhead is dual licensed, one clinical team treats both addiction and mental health at the same time. No separate referrals. No starting over with a different provider.

Family Support Services

Addiction affects the whole family. Treatment should too. Your individual therapist conducts family sessions weekly or bi-weekly, serving as a communication bridge between you and your family during treatment. Family education, support, and connections to external family resources are available across all program levels.

The therapist already knows your story, your progress, and your goals. Family sessions are built into your existing care, not outsourced to someone new.

What Sets Trailhead Apart

Dual licensed for substance use and mental health. Most outpatient treatment centers in New Hampshire are licensed for one or the other. Trailhead treats both with one team, one plan, one facility. No referral-outs for mental health if you are in SUD treatment, or the other way around.
