PHP provides structured, full-day outpatient treatment five days a week while you continue living at home. Trailhead is recovery-owned and operated, dual licensed by the State of New Hampshire for both addiction and mental health, and built around the belief that clinical care and holistic wellness belong in the same treatment plan.

What Is a Partial Hospitalization Program?

A partial hospitalization program is a structured outpatient level of care that provides intensive clinical treatment during the day while allowing participants to return home each evening. PHP sits between residential/inpatient treatment and intensive outpatient (IOP) on the care continuum. It delivers the clinical intensity of inpatient programming without requiring an overnight stay.

Daily Schedule

What a Typical Day Looks Like

Your day at Trailhead runs from 9:00 AM to 3:30 PM, five days a week, with a minimum of 5 clinical hours per day as required by New Hampshire regulations. The schedule alternates between clinical sessions and holistic programming so that each day addresses both the clinical and wellness dimensions of recovery.

Morning

Group therapy opens the day. Sessions are led by licensed clinicians and focus on skill-building, processing, and peer support. Morning also includes time for individual therapy sessions with your assigned therapist.

Midday

Lunch is provided on-site. The break gives you time to decompress between sessions.

Afternoon

Afternoon programming includes holistic sessions (yoga, breathwork, meditation, sound therapy, art therapy, ear acupuncture, or grounding techniques), case management check-ins, and additional group or individual work depending on your treatment plan.

Your schedule adjusts as your treatment progresses. The structure stays consistent enough to build daily rhythm, and flexible enough to respond to what you actually need week to week.

What PHP Includes at Trailhead

Individual Therapy

You receive a dedicated individual therapist assigned on day one. That therapist stays with you through your entire course of treatment, including if you step down to IOP or outpatient care. No handoffs, no repeating your story to someone new.

Group Therapy

Daily group sessions led by licensed clinicians. Topics include coping skills, psychoeducation, relapse prevention, and processing. Group work is the backbone of PHP programming and where most participants build the peer connections that support long-term recovery.

Medication-Assisted Treatment (MAT)

Trailhead offers injectable medication-assisted treatment including Vivitrol, Sublocade, and Brixadi for opioid and alcohol use disorders. MAT is integrated into your treatment plan alongside therapy and holistic care.

Case Management

Your case manager coordinates services beyond the clinical room: housing navigation, financial applications, sober living partnerships, and connection to community resources. Case management starts on day one alongside your therapist assignment.

Holistic Programming

Yoga, reiki, sound therapy, art therapy, ear acupuncture, breathwork, meditation, and grounding techniques are built into the daily clinical schedule at Trailhead. These are not optional add-ons or separate wellness programs. Co-Founder and COO Caitlin DiBlasi (BS, RN, CARC), a former yoga instructor and marathoner, designed the holistic component to address the stress, anxiety, and emotional dysregulation that clinical sessions alone do not always reach.

Family Sessions

Your individual therapist conducts family sessions weekly or bi-weekly based on clinical need. Because your therapist already knows your history, goals, and progress, family sessions are grounded in context rather than starting cold with a separate provider.

Women-Only Track

Women-Only PHP Track

Trailhead offers a women-only track within PHP, the first of its kind in New Hampshire outpatient treatment. The clinical curriculum mirrors the mixed-gender program with dedicated focus on trauma, codependency, and empowerment in a space where participants share common experiences.

Women-specific group sessions create room for topics that surface differently in mixed-gender settings. The structure, schedule, and clinical standards remain the same. The environment shifts to support the work that many women in recovery need to do around relationships, boundaries, and identity.

The women-only track is also available within IOP.

Every PHP participant is assigned an individual therapist and a dedicated case manager on day one. Your therapist stays with you from intake through completion and into step-down care. That continuity means the person guiding your treatment at week eight is the same person who met you on your first day.

Alysia LaGambina, LICSW, Clinical Director, leads the clinical approach at Trailhead. Her background spans adult mental health and addiction treatment across outpatient therapy settings, with years of experience as a public health social worker. Her approach is holistic and trauma-informed, recognizing the relationship between mental health, environment, and lived experience.

Trailhead’s clinical team includes licensed, trauma-informed therapists throughout. Recovery experience is valued and present across staff, which means the people working with you understand recovery from the inside, not just from textbooks.

Step Down Options

Continuing Care After PHP

When your clinical team determines you are ready to step down from PHP, you transition to Intensive Outpatient (IOP) or outpatient care within the same program at Trailhead. Your individual therapist stays with you through step-down. You do not switch providers, start over at a new facility, or lose the therapeutic relationship you built during PHP.

This continuity is one of the most important factors in sustained recovery. Trailhead offers a full continuum of outpatient care: PHP, IOP, outpatient, and medication-assisted treatment. Each step down reduces the intensity of programming while maintaining the clinical foundation.
