Trailhead Treatment Center provides outpatient addiction and mental health treatment in Salem, NH for adults maintaining recovery while managing work, family, and daily life. Our outpatient program includes individual therapy sessions, medication management, and continued care services. Trailhead is recovery-owned and operated by Co-Founder and COO Caitlin DiBlasi (BS, RN, CARC) and Mike DiBlasi, and led clinically by Alysia LaGambina, LICSW.

Outpatient is the right fit if you are stepping down from intensive outpatient (IOP) or partial hospitalization (PHP), or if your clinical assessment shows you need structured individual support without group-intensive programming.

What Is Outpatient Treatment

What Outpatient Treatment Includes

Outpatient treatment at Trailhead Treatment Center is individual-focused care for substance use and mental health. You meet with your therapist on a flexible schedule for one-on-one sessions, receive medication management when clinically indicated, and stay connected to your recovery support system between appointments.

This level of care is built for two situations:

Continued care after IOP or PHP. You completed a higher level of care and your clinical team recommends stepping down. Your treatment doesn’t end. It adjusts.

Standalone outpatient support. Your clinical assessment (ASAM, LOCUS, K10) shows that individual therapy and medication management meet your current needs without group programming.

Trailhead holds dual licensure from the State of New Hampshire for both substance use disorder and mental health treatment. Your therapist treats both in the same program, at the same time. You do not get referred out for mental health care if you are in substance use treatment, or the other way around.

Trailhead offers a full continuum of outpatient care: PHP (9am-3:30pm, 5 days/week), IOP (9am-12:30pm, 5 days/week), and outpatient. Moving between levels happens based on your clinical progress, not arbitrary timelines.

This matters because recovery is not always linear. Some weeks are harder than others. If your clinical team identifies a need for more support, they can increase your level of care immediately within the same program. You keep your therapist, your treatment history stays intact, and you skip the intake process that comes with finding a new provider.

Standalone outpatient clinics cannot offer that. If things escalate, they refer you out. At Trailhead, stepping up is a clinical decision made by the team that already knows you.

Trailhead was founded by Caitlin and Mike DiBlasi. That firsthand experience shapes how the program operates, from the admissions process (designed to feel human, not bureaucratic) to the clinical environment (warm, nature-inspired, personal).

Caitlin brings a background as a BSN-prepared nurse, certified addiction recovery coach, and former yoga instructor. She designed the Holistic programming that integrates yoga, reiki, sound therapy, art therapy, ear acupuncture, breathwork, and meditation into clinical care. These are built into the treatment schedule, not offered as optional extras.

Clinical Director Alysia LaGambina, LICSW leads the clinical approach with a strong therapeutic foundation in adult mental health and addiction treatment across outpatient and public health settings. Her holistic, trauma-informed perspective recognizes how mental health, environment, and lived experience connect.

Insurance verification. Our admissions team verifies your benefits during the initial conversation and explains your coverage clearly.

Clinical assessment. A phone-based assessment using ASAM (substance use), LOCUS (mental health), and K10 (psychological distress) determines which level of care fits your situation.

Prior authorization. If your insurance requires it, we handle it. You do not make separate calls or chase paperwork.

Intake appointment. You come to our Salem, NH facility, meet your therapist and case manager, and begin treatment.

Most clients move from first call to first appointment within 24 to 48 hours. Weekend scheduling is available.
